Chronic Disease CDC Program

State Public Health Actions to Prevent and Control Diabetes, Heart Disease, Obesity and Associated Risk Factors and Promote School Health

Chronic Disease Program offers:

Living Well with Chronic Conditions (the Chronic Disease Self-Management Program or CDSMP) is a six-week workshop that provides tools for living a healthy life with chronic health conditions, including diabetes, arthritis, asthma and heart disease. Through weekly sessions, the workshop provides support for continuing normal daily activities and dealing with the emotions that chronic conditions may bring about.

The Gold Sneaker Initiative was developed to enhance policy related to health and wellness within licensed child care facilities across Tennessee. The Chronic Disease Program provides free training to any child care facility interested in becoming Gold Sneaker certified.

Promote and assist in sponsoring local Farmers’ Markets that accept SNAP benefits
The Chronic Disease Program has access to a number of grant opportunities for organizations and or communities that might be interested in pursuing
