Birth or Death Certificates can be requested between 8am-4pm Monday thru Thursday, closed for lunch 11:30pm – 12:30pm; Friday hours 8am-4pm, closed for lunch 11:30am -12:30pm. Closed on Holidays.

We issue death certificates for deaths which occurred anywhere in Tennessee within the past 50 years. Certified copies of death certificates are available at the Blountville office ONLY.

In addition, we issue long form birth certificates for births which occurred anywhere in Tennessee in the last 100 years. The certified long form birth certificates are available at the Blountville office ONLY.

Birth Certificates

Sullivan County Regional Health Department’s Blountville Office issues Long Form Birth Certificates for births which occurred anywhere in Tennessee in the last 100 years. The certified long form birth certificates are available at the Blountville office ONLY.

Identification Requirements

The request must be made by the person named on the certificate, or a parent, guardian, child or spouse of the named person. Most requests for certified copies can be handled while you wait.

Information needed includes: name, date of birth, mother and father’s names, mother’s maiden name, and persons place of birth.

Cost $15.00 per certificate.  Visa & MasterCard are accepted. Checks should be made payable to Sullivan County Regional Health Department.

Birth Certificate Application

If you are not married when your child is born, your child does not have a legal father. This service allows the father’s name to be placed on the birth certificate for a child born out of wedlock.

The father’s name may be added to a birth certificate by completing a Voluntary Acknowledgment of Paternity, if all of the following are true:

  1. The child has not yet reached his/her 19th birthday.
  2. The mother was not married at the time the child was born or within 300 days prior to the child’s birth.
  3. Both the biological mother and the father want to add his name to the birth certificate.
  4. There is no father’s name on the birth certificate.

If the answer to any of the above questions is false, a court order would be required to add the father’s name to the birth certificate.

This service is by appointment only. Please call for an appointment. Appointments are available Monday through Thursday.

Death certificates available to eligible individuals for those who have expired within the State of Tennessee in the last 50 years. Death Certificates are available at the Blountville Office ONLY.   Identification Requirements

Cost is $15.00 for each certificate. Visa & MasterCard accepted. Check should be made to Sullivan County Regional Health Department.

Information needed: name, sex, date of death, place of death, funeral home, relationship to the decedent, purpose of copy. Photo ID and other verifying information will be necessary if cause of death is to show on the certificate.
