Community Health Access and Navigation in Tennessee (CHANT)

Connecting families to medical reimbursement and essential resources for children with disabilities

What is CHANT?

Our CHANT program is an integrated model of care coordination. The program provides enhanced patient-centered engagement, assistance with navigating complex systems, and care coordination of medical and social service needs.

When you call to connect with a Care Coordinator they will help you navigate our Community Partners and establish a connection with your family to help assist you in setting goals to help meet your everyday needs. We can assist your family with the following:

  • Applying for TNCare
  • Locating Primary Care
  • Child Health and Development
  • Acquiring Pack-n-Plays
  • And other incentives

Who is eligible?

Individuals eligible for CHANT include:

  • Pregnant and postpartum adolescents and women
  • Children (Birth - 21 years)
  • Children and Youth with Special Healthcare Needs (Birth - 21 years)

We also offer assistance through our CSS program for families that have a child or children with special needs from birth until their 21st birthday.
